Please complete this form to borrow MMHA equipment . We will notify you with pick up instructions when the equipment is ready for pick up.

Payment Details

To sign out IPads and/or Goalie Equipment for the season, a credit card must be entered as security. If an IPad is not returned at end of season, a $350 charge will be incurred. If a set of Goalie equipment is not returned at end of season, a $500 charge will be incurred.

Office Use Only

I acknowledge that I am personally responsible for returning borrowed equipment to the MMHA office, or in the instance of goalie equipment, to Play It Again Sports in Milton, within a week of the end of the season. If equipment is not returned, my credit card will be charged.
