**PLEASE NOTE PTS WILL BE PREPARED IN ACCORDANCE TO HOCKEY CANADA AND OHF GUIDELINES**

If you have more than one child who needs a form(s), please fill out a separate request form for each child.

If you are requesting a PTS for Junior please fill out a separate form for each team the player is trying out for.

Once the form has been signed by Milton, we will email you to pick up. You will need to bring it with you to your tryout.
